Driver qualification file checklist
One file per driver, including you if you're the only driver. Auditors check these first.
FMCSA requires a driver qualification (DQ) file for every driver of a commercial motor vehicle (49 CFR Part 391), even owner-operators driving for their own authority. Keep it for as long as the driver works for you and 3 years after.

Questions carriers ask
Do I need a DQ file for myself?
Yes. Owner-operators driving under their own authority are both the carrier and the driver, so you keep a file on yourself.
Where do drug test results go?
In a separate, confidential drug and alcohol file, not in the DQ file.
